Some of the first decisions that parents make about their newborn surround the screenings and procedures offered within the first few days of birth. The document handout for parents covers all aspects of a parent's choices, including:
- Newborn Metabolic Screen
- Critical Congenital Heart Disease Screen
- Hearing Screen
- Newborn Eye Prophylaxis
I think it is important for parents to have all the information in one spot about newborn screening and procedures, instead of piecemealed one document at a time, and this document handout does that. The consent/refusal document also addresses all four screenings and procedures on one page. However, the parent handout or consent/refusal does not address each state's midwifery protocol or mandates. I highly recommend customizing the document to reflect your midwifery protocols.
Each midwife should also have handouts from the state they live in for each screening and procedure. Parents need as much information as possible to help them make decisions about their newborn.
What You Get:
- Two-page "Newborn Screening and Procedures" handout for parents with references (1 PDF fle).
- One-page informed consent, "Consent/Refusal for Newborn Screening and Procedures" for signature, which is formatted to upload to Mobile Midwife or Client Care (1 PDF file, 1 PNG file), or use as a paper file.
- These black and white documents are meant to be printed on 8.5 X 11" paper.
- Please note: You will NOT be receiving a physical copy of these items.
CUSTOMIZATION
You have the option of personalizing the files you purchase to fit your practice for an additional $45 ($15 per document). I take extra care to ensure your documents look professional. Any logo or image you send MUST be high resolution (300 dpi).
Once I receive your order, I will contact you to arrange details. Proceed to checkout as usual, and download the documents.
Customization of Handout:
- Practice name, midwife name, logo at top.
- Footer info, such as contact information.
- Choice of singular or plural midwives, for example, "I offer," "We offer."
- Your state or practice guidelines, no more than one paragraph.

- Small header at top with your practice name or personal name
- Footer info, such as contact information.
- Choice of singular or plural midwives.
- Your state or practice guidelines.
Please allow 10-14 business days after I receive your information for the completion of your customization.
