The 24-28 week labs help identify problems that a woman might have during this time in her pregnancy. In this informed consent document, labs are either required or optional. The CBC is an important part of routine testing, as is an antibody screen for Rh negative mothers. The gestational diabetes document gives parents a clear idea of their risk factors and options to accept or decline gestational diabetes screening.

You have the option of personalizing the files you purchase to fit your practice for an additional $45. I take extra care to ensure your documents look professional. Any logo or image you send MUST be high resolution (300 dpi).
